It is definitely true that it would be ideal to have all name servers
be in the domain.  Part of the problem is, that relatively few domains
cover enough ground to legitimately have more than one name server in
disparate locations, or sometimes not even one.  Yet some of those may
have legitimate reasons to want their domains resolved on the public
Internet, even if one name server is down.  [Not to mention that many
registrars still require >1 name server.]
