Why are they asking for both the DNS server and the IP?

NS records in DNS always point to host names, not IP addresses, so you have
to provide the nameserver names.

The registrar also asks for the IP address in case they need to create a
glue record (this is needed if the nameserver's name is in the domain being
delegated to it).
