Notify & Refresh Suddendly Not Working

Check your log for messages that say "NOTIFY(SOA) for zone already xferring
(<zone name>)".  Sometimes named doesn't clear the transferring flag on a
zone, so it won't start up another transfer for it.

The only solution we've found for this is to restart named.  This is
another problem I think we're going to try to get Nominom to address for
us.
