There was a bug very similar to this in earlier DHCP 4 versions.  I
am surprised there's still a problem...

It probably has more to do if there is a pre-existing lease structure
that has expired or not than anything else you've brought up.  The
heap referred to is used to organize active leases for expiration in
an O(log(n)) manner (for n=number of active leases).
