Ah, OK.  So, after doing a bit more digging, it would appear that the 
following syntax *should* work under 4.2.0 (Calix example):

host john {
    host-identifier option agent.circuit-id "Ont:N71-1-1-1-32-OntEth1-1";
    fixed-address 10.0.0.200;
}

Might be time to upgrade.
