This seems to be exceedingly common for in-addr.arpa zones - don't
people care about RDNS?

A lame server is one which has one or more zones delegated to it, but
it does not answer authoratively for those zones. For example, I
register a domain DUMMYDOMAIN.ORG, and point it to my three name
servers. However, I forget to reload the configuration on one of the
secondaries so it does not pull the zone from the master, and thus
cannot answer authoratively. When asked for DUMMYDOMAIN.ORG, it
responds with an unauthorative (aa flag cleared in response) referral
to the ORG gTLD.

Then that server will be considered lame for the DUMMYDOMAIN.ORG zone.
