The O'Reilly _DNS_and_BIND_ book covers this fairly extensively for
BIND versions up through 8.1.2.

Unfortunately, debug output tends to change from release to release, so
any documentation tends to get outdated. The debug output of BIND 9, for
instance, is likely to be significantly different from BIND 8 because
it's a total rewrite.
