i am very interested in finding any bug in our form.
so far it seems to have worked fine with hundreds of entries.
you pasted the output exactly as it is, pushed send, and forwarded
the confirming email back to granitecanyon and got the 
final confirmation email back and waited a day ?
if you then still get errors, could you please post the entry ?
