I don't think there were any failover changes introduced in
3.0.5.

Yeah, I can't explain that.

If that showed up on a server that was running 3.0.4 from start to
finish, that's pretty unexplainable.

If it's a server that was upgraded from an older version, you might
want to consider faulting one server so it recovers the lease db
from the peer.
