How to setup DNS to work without the "www" on shared hosting?

No can do - a domain name can not have a CNAME and other data.

Presumably domain.com already got nameservers, and an SOA, and
maybe an MX, so a CNAME just isn't allowed.

You really have to use an A record here.

domain.com. A 123.123.123.123
